Guidance on 2D-Axisymmetric Modelling Approach

I'm trying to model a schematic which looks like this (in the picture; it makes everything much clearer).
Everything has been set-up, boundary conditions, flux, and all. But when I attempt to compute, I get the "Undefined Value found in Equation Residual Vector" error, and most glaringly, the boundary between the 2 domains always comes in.
It seems like whenever I model a domain that is off-axis, it is unable to converge. A model which would normally converge if it were centered around the axis fails when I shift it off (like in Flow 2). As you can see, Flow 2 has to be off-axis.
Under this circumstance, I would very much appreciate any suggestions on how to mitigate this problem, and whether any coupling is needed for both inner and outer domains, considering that there is a flux across from one to the other.
