How to set the special boundary condition?

Hello I am using the PDE module. I want to set some special boundary condition. It is shown in the Fig. P is the parameter to be solved. C is the constant value. Notice that the equation in this figure is based on the corrensponding points. Two endpoints of green or red line are the corrensponding points. Many thanks for your reply.
