Challenge in initialization of hole in 3d

I want to initialize a hole in three-dimensional, that is, an hemispherical hole as in "my goal.jpg". But the result I am getting is not what I am expecting, even though my equation is correct (capture_sphere). See "3D Plot Group 9" in mph model. I tried using flc2hs to scale it, yet it did not work. See "3D Plot Group 7" in the model. Please, what can I do? Please, find the attached Comsol model and pictures. Thank you so much.
