Fracture boundary conditions, weak form

Hello,
I am building a porous media model containing discrete fracture by using Coefficient PDE form. However, I am confused how to set the boundary conditions at the interior fracture. It is said in the reference that the weak form should be used at the fracture. Can anyone provide related information for me?
