Why do the general and coefficient PDE form have their current form?

Especially, I'm wondering about the boundary conditions. There are kinda couplings between the formulations of Dirichlet and Neumann boundary conditions by R (e.g., see page 258 in COMSOL Multiphysics Modeling Guide for version 3.5a). Why is this necessary?
